How to get started
- 01Reach out. Call or send us a message. We answer your questions, explain your options and verify your insurance coverage.
- 02We coordinate with your doctor. Home health care is ordered by a physician. We work directly with your doctor or hospital discharge planner to put everything in place.
- 03Your first visit. A nurse or therapist visits your home, completes a full assessment and builds a personal plan of care around your goals.
- 04Care that moves you forward. Your team visits on a steady schedule, tracks your progress and keeps your physician and family informed along the way.
